About the proposal

Funding: $20,415,530 in funding to support the attachment of 41,534 patients across Middlesex London by 2027 ($1,505,530 in one-time start-up costs and $18,910,000 in recurrent revenue costs).

Partners: over 18 collaborative partners, including four primary care organizations (Thames Valley FHT, London FHT, London InterCommunity Health Centre, and Health Zone NP Led Clinic). 

Priority: communities with high rates of unattachment and embeds the core principles of a Health Home model. All participants in the plan will be expected to work closely with Health Care Connect to attach individuals on that list. 

The proposal includes: 

  • Increased primary care capacity through team-based care 
  • Navigation and Referral Support Team 
  • Supported Attachment Team 
  • Supportive technology 
  • Specialist Sessional Fees 
  • Community Care supports 

Staffing numbers: to reach the target population, the proposal indicates that 150 new staff will need to be hired, including 87.6 clinical staff (full and part-time) and 61.8 admin/management staff (full and part-time hours). These staffing numbers are based on 100% of the funding being awarded. However, based on previous experience, it is likely that the proposal will be partially funded.  

What happens next?

Once we receive notice of how much we are awarded, the implementation phase will begin. We will then re-engage with partners and modify the staffing expectations based on any funding limitations.
