Strategic Goals

(2025-2026 and 2026-2027)

1. Enable and support primary care expansion/access and attachment (per provincial timelines)

  • 100% of Middlesex London Health Care Connect backlog patients attached by March 2026 (Spring 2026)
  • 100% of total Middlesex London unattached, attached by March 2029
  • 100% of interested providers will be supported and connected with future IPCT proposal opportunities
  • Recruit six or more new family physicians, per year, to Middlesex London

2. Grow and strengthen MLPCN engagement

  • Increase confirmed membership by 50% by March 2026
  • Grow MLPCN’s confirmed membership by an additional 50% by March 2027
  • 100% registration and attendance at the 2025 Primary Care Symposium
  • 100% of co-design and system level initiatives involve participation by MLPCN engaged members
  • Participation in events by our partners (London District Academy of Medicine and Department of Family Medicine) three events per year attended by 50+ confirmed MLPCN members

3. Partner on referral management improvements

  • 100% of partners interested in exploring referral management process improvement/ centralized referral management will be supported by MLPCN in co-design
  • Initiate four referral improvement opportunities per year
  • 25% increase in eReferral sign-ups, per year, for primary care providers
