Older adults living with living dementia and multiple chronic conditions often face new or escalating behavioural symptoms that challenge both care planning and the well-being of family caregivers. This ECHO session is designed for primary care and home care providers working together to support seniors in their homes. Join us to explore integrated, person- and family-centred strategies for managing responsive behaviours in older adults with dementia. Learn how to coordinate care across sectors, engage caregivers in shared decision-making, and build trust-based care plans that reflect individual values and evolving needs. Through a clinical case and panel discussion, this session highlights real-world examples of how collaborative care planning can improve outcomes for individuals with dementia and complex care needs. Learning Objectives: • Identify common responsive behaviours for those living with dementia and determine when to involve interdisciplinary and specialized care teams in collaborative care planning. • Apply approaches to engage individuals living with dementia and their family caregivers in shared decision-making that builds trust and reflects personal values and preferences. • Reflect on clinical experiences and lessons learned from collaborative care planning across home care, primary care, and community support teams.
