Community paramedicine is a 24/7/365 non-emergent mobile response team with specialized training designed to work in collaboration with you to provide a range of community-based primary care services in the comfort of a patient’s home.
The CPPCS Medical Directives apply to paramedics who are authorized to operate as a MLPS Community Paramedic and provide patient care. Delegation of these Medical Directives is under the authorization of the Community Paramedicine Medical Director, Middlesex-London Paramedic Service, in conjunction with the patient’s primary care provider. Community Paramedicine FAQs
What can Community Paramedics do?
- Primary assessments inclusive of, but not limited to blood pressure, heart rate, respiratory rate, oxygen saturation, ETCO2, blood glucose, weight, temperature, 4 lead ECG, in-depth physical assessment;
- Advanced assessments inclusive of, but not limited to 12 &15 lead ECGs, Palliative Performance Scale, Edmonton Symptom Assessment System, Confusion Assessment Method, cognitive assessment;
- Remote monitoring inclusive of blood pressure, heart rate, oxygen saturation, blood glucose;
- Chronic condition management inclusive of, but not limited to: COPD, HF, DM, osteoporosis, shingles, falls, pain, febrile, nausea/vomiting;
- Palliative care acute pain and symptom management;
- Wound care management;
- Suture/staple removal;
- Point-of-care testing (CBC, HbA1c, CHEM 8, PT/INR, Lactate, Urinalysis, COVID-19 testing);
- Point-of-care ultrasound detection of: abdominal aortic aneurysms, pericardial effusion, global cardiac function, abdominal peritoneal free fluid, pneumothorax, pleural effusion, bladder volume (LTC Only);
- IV access and fluid therapy;
- IV antibiotic medication administration;
- Influenza vaccine administration;
- Rx medication administration;
- Motivational coaching, education, and self-management guidelines;
- Medication compliance;
- Collaborative care coordination with health care providers;
- Referrals to community resources and programs;
- Provide timely reports to support an informed clinical decision on patient management; AND
- 911 treatment and transport, if within the patient’s goals of care.
Criteria for referral
- Those on the wait list for Long-Term Care
- Those who have been assessed as eligible for Long-Term Care by a Home Community Care Support Service Care Coordinator
- Those who are soon to be eligible for Long-Term Care
- Those who are on the Long-Term Care crisis wait list
- Individuals with specific circumstances or conditions that would benefit from Community Paramedicine services
- Complete the Community Paramedicine Referral form. NOTE: The Community Paramedicine Referral Form is to be completed by or in consultation with the primary care provider. All referrals MUST be accompanied by a signed Provider Sign-Off Form.
